Code review guidance

This is guidance for reviewers — not a normative requirement and not a landing gate. It captures recurring pitfalls so the knowledge is written down and reusable (by a human reviewer, or by an automated reviewer that reads a page as context) instead of living only in individual reviewers’ heads.
